How Hair is Lost
Alopecia is the medical term for hair loss. Androgenetic alopecia (AGA), commonly referred to as male pattern baldness, is the most frequent cause of hair loss in men (AGA can also affect women, resulting in female pattern baldness). Patients with AGA lose hair in a well-defined pattern, affecting the temples, top, and crown portions of the head. Often, the condition is hereditary; patients inherit hair follicles that are highly sensitive to dihydrotestosterone (DHT), a derivative of the male hormone, testosterone. Over time, DHT shrinks sensitive hair follicles until they can no longer produce aesthetically pleasing hair. Though AGA is by far the most common cause of severe baldness, different degrees of hair loss can result from a wide variety of other conditions as well.
Even in patients with hereditary AGA, the hairs at the sides and back of the head are typically DHT resistant. Surgical hair restoration, or hair transplantation, involves harvesting small donor samples of DHT resistant hair follicles, and then transplanting them into balding areas. The donor samples are taken from the patient’s own hair follicles at the sides and back of the head. If you happen to have an identical twin, they can donate hair follicles as well.
Hair transplantation is a delicate procedure performed in our office under local anesthesia. The technique involves removing a miniscule layer of hair-bearing skin from the patient’s donor area, then transplanting the graft to an area lacking healthy hair follicles. The donor area is then sutured, which may leave a thin linear scar that, in most cases, is immediately concealed by hair. The removed grafts are microscopically divided into separate follicle graft units containing 1-4 follicles each (depending on the grafts’ intended locations). Micro-incisions are made in a carefully planned fashion to mimic the natural growth pattern of the patient’s hair. Since the incisions and grafts are small, bandages are not necessary. Some of the transplanted hairs may begin growing quickly, though others may take up to 24 weeks before exhibiting noticeable growth. Increased hair density can be accomplished through subsequent treatments, but most patients are satisfied with just a single procedure.
